Job Title: Primary Healthcare CoordinatorJob Type: PermanentJob Location: Abu DhabiJob Description:Support care delivery to patientsMedical Centre Operation coordinationCommunicate and provide resources to patients and caregivers about patients conditions and treatment requirementsRecognize and appropriately manage challenging behaviors of patients and caregiversLiaise and follow through with patients and all health and social care providers to keep everyone informed and updatedOrganize patient information for efficient data transfer to relevant internal staff and external providersReceive and collate information from new existing and transfer patientsLiaise with other providers to ensure patients at risk are monitored adequatelyConduct checks on data input to ensure quality and consistencyGenerate reports for population health managementCoordinate among multidisciplinary teams in the providers settingLiaise with clinical and nonclinical staff for effective patient managementOrganize and manage minutes of internal meetings as neededFollowup and tracking of preventive screening results to ensure the timeliness and completeness of followup as specifications and carepathwayQualifications:Preferred Qualifications:Diploma or equivalent certificate in Healthrelated fields (e.g. Health Care Administration Public Health Health Education) Social Sciences or Business AdministrationBachelors degree in Healthrelated fields (e.g. Health Care Administration Public Health Health Education) Social Sciences or Business AdministrationPreferred Experience:Minimum 2 years of experience at a healthcare provider (e.g. hospital outpatient clinic) or with a healthcare professionalMedical center operation and coordinationQuality experience in JAWDA TASNEEF KPIsExperience in administrative duties and use of databasesExperience in primary care services (such as chronic disease management and preventive services)Knowledge of medical terminologySkill Set and Attributes:Excellent verbal and written communication skills in English and ArabicStrong organizational and documentation skills and attention to detailStrong critical thinking and problemsolving skillsProficiency in the use of common word processing presentation spreadsheet and email toolsData analysis and management skillsSkills Hub is an online recruitment portal based in Abu Dhabi.Skills Hub is your onestop shop for job searching success.
